久しぶりの記事投稿となってしまいました。
一度遠ざかってしまうと、なかなか再開も難しくなるものです。新年度ということもありますが、私の生活スタイルも大きく変化しており、時間を見つけて少しずつ投稿していきたいと思います。
乳児の鵞口瘡（口腔カンジダ症）の相談を受けました。基本的には自然治癒が期待できるものですが、なかなか改善みられないため、治療について検討しています。少し調べてみることにしました。
DynaMedを"thrush"で検索。"Oral candidiasis in infants"の治療の項には以下の記載があるのみでした。

UpToDateでは、"Treatment of oropharyngeal and esophageal candidiasis"に以下のような記載があります。

Nystatin swish and swallow (400,000 to 600,000 units four times daily) has been used for many years, but is not always palatable to patients [4]. It also contains sucrose, which can cause dental caries when used over prolonged time periods [5]. The administration of clotrimazole troches (one 10-mg troche dissolved slowly five times daily) is effective, but is difficult to adhere to due to frequent dosing [4]. For topical agents, successful therapy depends on adequate contact time between the agent and the oral mucosa [5].
ナイスタチンが長年使われているが、万人の口に合うわけではない、スクロースを含有するため長期間使用すると虫歯の原因となる、などのデメリットがあると記載されています。
PubMed Clinical Queriesから"Oral candidiasis infants"で検索したところ、以下のレビューが検索されましたので、読んでみることにしました。

Nystatin oral suspension is a safe first-line therapy; fluconazole is more effective (strength of recommendation [SOR]: B, 1 small randomized controlled trial [RCT]) but has not been approved by the Food and Drug Administration (FDA) for use in immunocompetent infants. Miconazole oral gel is also more effective than nystatin suspension, but is not commercially available in the united states (SOR: B, one small RCT). Gentian violet may be effective, but it stains skin and clothes and is associated with mucosal ulceration (SOR: B, 1 small retrospective cohort study).
要約にある通り、ナイスタチンは第一選択薬であるが、フルコナゾール、ミコナゾールはより効果的とのことでした。

4週間以内の再発率（12.4%, 13.0%）、副作用（4.5%, 3.5%）には差がなかった。
ナイスタチンよりミコナゾール口腔用ゲルのほうが早く治る、という結果のようです。自然治癒が期待できますので、さらに追跡していくと、いずれ両群の治癒率は同等になるのでしょう。
